Job Title:        Diabetes Education Team Coordinator

Location:        Suffolk County, Riverhead, NY

Position Summary:

 

Responsible for managing CCE programs in diabetes education, including county-funded diabetes education and self-management program and New York State funded diabetes prevention and control programs. Supervise, train, advise and provide leadership to diabetes education staff, including registered dieticians, registered nurses, program educators and assistants and support staff.  Assist with generating revenue to carry out programs.   Participate in collaborative program efforts with public, private and non-profit agencies.  In conjunction with the Program Director and Family and Consumer Sciences Program Committee, provide initiative to develop, plan, implement and evaluate educational programs in diabetes education for adults and youth, including direct service in health centers as needed. Represent the Association to the public, volunteers, community leaders, schools, agency personnel, government officials and Cornell University.  Responsible for providing in-depth disciplinary support and input into program, evaluation, grants/contracts, marketing, research, and specific program initiatives and efforts connected to Cornell Faculty, program and work teams.

 

Responsibilities and Essential Functions:

Issue Area Leadership Program Development and Priority Setting  (10%)

Working with program committees, advisory groups and program participants, identify local program needs in the areas of diabetes, nutrition and health issues. Prepare plan of work and implementation strategies. Ensure that program direction and related activities are coordinated within the overall plan-of-work and the mission of Cornell Cooperative Extension. Develop and enhance relationships with appropriate agencies, organizations, industries and community leaders. Participate in multi-disciplinary team efforts to address priority issues. Identify and structure programs that address community and individual needs that consider the diversity and cultural context of the community. Develop strategies to involve multi-cultural audiences in program efforts. Assess the need for and involve volunteer participation.

 

Program Management and Coordination (30%)

Provide overall leadership and management for Association’s diabetes education programs and outreach, including the direct supervision and professional development of diabetes educators and support staff. Prepare reports on program progress and accomplishments of association programming for local, state and federal use. Train, supervise and evaluate staff.  Support association and statewide marketing efforts.

 

 

Subject Matter Resource and Program Delivery (30%)

Develop networks of volunteers and/or professionals to extend the program implementation efforts. Provide educational programming to targeted Long Island audiences in diabetes education. Serve as a key subject matter resource in the area of diabetes prevention and control for the FCS program advisory committee, County Departments, Association staff and clientele including schools, agencies and other groups on a local, regional and/or statewide basis as identified.

 

Research and Program Evaluation (10%)

Maintain linkages with Cornell faculty and program administrators as appropriate. This may include conducting applied research in conjunction with faculty and helping to develop written materials for publication or toward development of video or computer programs. Evaluate and analyze programs in diabetes education and make recommendations for change in emphasis and /or improvement. Communicate evaluation results, findings and recommendations to staff, program and board committee members, clientele, administration and Cornell faculty.

 

Administrative Management (Finance, Budget) (10%)

Monitor programs to ensure that educational content conforms to the policies and objectives of the Association and Cornell University. Prepare reports on program progress and accomplishments. Compile budget data and written reports on program finances and work plans. Seek out, prepare and submit proposals to secure additional funding.

 

EEO/EPO (5%)

Develop strategies to involve multi-cultural audiences in program efforts. Reach out to diverse audiences. In the recruitment of staff develop and implement a plan to ensure all searches reach out and include a diverse pool. Appreciate and embrace diversity in all interactions with clientele, staff, volunteers and the public.

 

Professional Improvement and other duties as assigned (5%)

In cooperation with supervisor, jointly develop and pursue a professional development plan as a means to increase competencies related to position accountabilities to address change in programming and association priorities. Attend in-service education as needed, in relation to program responsibilities. Participate in professional associations.

 

Qualifications

 

Master's degree in Nutrition, Public Health or related field plus minimum of two years related experience required.  Certified Diabetes Educator (C.D.E.) preferred.  Advanced knowledge of principles, theory and research in the area of diabetes education, health and nutrition strongly preferred. At least two years progressively responsible experience in program administration and staff supervision desirable.

§         Proven ability to utilize innovative program delivery methods to reach individuals and groups Responsibilities require attendance at programs and meetings, which may be scheduled on evenings and weekends

§         Ability to meet travel requirements for position; travel outside of Suffolk County may be required

§         Ability to conduct programs, transport equipment/supplies to multiple sites throughout the county

§         Ability to communicate in Spanish desirable
